Institute 4 Discipleship Course Application

Please note: all catalyst courses are non-accredited.

Applying for your Catalyst Course

To apply for your Catalyst course, you will need to:

1. Complete this application form

2. Provide the following supporting evidence on page 6:

- Photo ID - e.g., driver's license, passport, ID card (Please ensure all documents are either PDF, png, or jpeg)

- NSW working with children check number (WWCC)* and expiry date (if you have one)

- Resume (Click here for a resume template if you do not already have a resume)

3. Pay your deposit via credit card - this is non-refundable

4. You will be emailed a comprehensive medical form to complete and submit

5. Your chosen referee (on page 3) will be emailed a reference form to complete. They will have 7 days to complete this.

6. Once all supporting evidence has been received, they will be reviewed.

7. If you are accepted into the course, you will be enrolled, and you will receive an invoice for the remaining amount (payable by the due date on the invoice)

*Note - If you do not have your WWCC, Catalyst will assist you in getting this done during the first two weeks of the course.

Referee Details

The application process for Catalyst requires a reference form to be completed by a suitable referee. Avondale will email the reference form directly to your nominated referee's email address.

A suitable referee should not be related to you and be one of the following:

Church pastor

Chaplain

Youth leader

Pathfinder leader

Church elder

NOTE: You must inform your referee that they will receive a reference form from Avondale via email, and they may be contacted to discuss any of the information in the form.

Wilderness Experience

Catalyst includes a wilderness experience week in Bungonia, which includes activities such as camping, hiking, caving, canyoning and abseiling. No prior experience is required.
